The following steps will assist you in filling out the proper paperwork to make your first order. You will need a printer to complete this proccess.
Step 1: PDF Files
If you have an application that can view and print PDF files, go ahead to step 2.

Step 4: Make Your First Order!
Order Form: You can open this document, type directly into the file and save it! Once your form is completed, email it to your salesperson or print it and fax it to 541-485-7141. Save copies of this file to keep track of your orders without cluttering your desk with paperwork.
